Make LOD for trees/buildings/objects/STG configurable.

Now based on /sim/rendering/static-lod/rough.
This commit removes a large number of range
settings that should in general be set by the user.

Workaround for "AI Models invisible"
...
expose the ai-range-mode flag to the static-lod dialog.
PagedLOD culling based on distance seems to be more reliable than that based
on projected screen size.
